Steps to request an appointment: 

1. Check out our openings and pick the appointment for the SERVICE needed that works best for you.  For help, see “Tips for Scheduling” below. 
2. Login to your Raintree Patient Portal and message our team your top choices.
   • Call us at 763-755-4275, option 1, if you need an invitation to create your Patient Portal account or assistance logging in.
3. We’ll confirm your appointment request(s) and reserve them on our schedule.

Tips for Scheduling

• Only request appointment spots that match your therapist’s recommendations.
  • SP= Speech Therapy (Speech Therapy patients can request any appointment that has “SP” in the type.)
  • OT = Occupational Therapy (Occupational Therapy patients can request any appointment that has “OT” in the type.)
  • PT = Physical Therapy
  • Myo = Myofunctional Therapy
• Our patients can schedule therapy at multiple clinic locations with multiple providers. Grab an appointment time/location that works for you!
